This waiver must be read and signed before receiving any permanent jewelry service from Alayé Permanent Jewelry. It is incorporated into and forms part of our Terms & Conditions.

Acknowledgment of Risks

By receiving permanent jewelry services, I acknowledge and accept that:

Permanent jewelry is not indestructible and may break due to normal wear, accidental snagging, pressure, or misuse.
The welding process produces a brief, low-energy spark. No needle is used and no heat is applied directly to the skin.
I must inform Alayé Permanent Jewelry before service if I have a pacemaker, defibrillator, or any other electrical medical device.
Some individuals may experience adverse skin reactions to certain metals or alloys, including redness, itching, swelling, irritation, or contact dermatitis, and I must disclose any known metal allergies or sensitivities before service.
I voluntarily assume all risks associated with this service, including but not limited to chain breakage, metal sensitivity, allergic or adverse skin reactions, accidental damage, and loss.

Health & Sensitivity Disclosure

Please disclose any known metal allergies, skin sensitivities, or relevant medical conditions below. This information is kept confidential and used only to help provide safe service.

Release of Liability

I release Alayé Permanent Jewelry, its owners, employees and contractors from any and all liability, claims, or causes of action arising from this service, to the fullest extent permitted by law, including but not limited to indirect, incidental, or consequential damages, loss or damage of jewelry after it leaves the appointment or event, and allergic reactions when known sensitivities were not disclosed.

Aftercare Acknowledgment

I understand that proper aftercare (avoiding excessive pressure, snagging, and harsh chemicals) helps my piece last, and that Alayé Permanent Jewelry is not responsible for damage caused by misuse, accidents, or failure to follow aftercare guidance.

Photo & Media Consent

I consent to being photographed during my appointment or event for marketing and social media purposes. I would prefer not to be photographed.

Minors

Clients under the age of 18 must be accompanied by a parent or legal guardian, who provides consent and signs below on the minor's behalf.
